In a long-awaited document, Sir Wyn Williams hit out at bosses at the Post Office, saying they should have known the faulty software could lead to errors.
Around 1,000 postmasters were prosecuted and convicted after the IT system made it look like cash was missing from their branches. This had a "disastrous" impact on those affected - including family members of those wrongly accused.
Tragically he said it is a "real possibility" that 13 victims took their own lives, while the inquiry heard evidence that a further 59 had contemplated doing so. Lead campaigner Jo Hamilton said the Inquiry report “shows the full scale of the horror that they unleashed on us.”

'I was spiked by my best friend - it made me an alcoholic at 23'Our Lifestyle team spoke exclusively with Sam Thomas, who’s 'best friend'spiked his drink with vodka, which led the then-23-year-old into a downward spiral. Becoming severely alcohol dependent, by the time he was 26, he would be drinking a bottle of wine a night, and isolating himself away so he could drink alone.
But with a successful career, regularly going to the gym and masking his drinking, Sam, a writer, seemed to be fine. Until one day the drink took over his gym hobby, leading him down a dangerous path. Visiting A&E over thirty times, being detained by the police, admitted to a psychiatric hospital four times and visiting a medical detox centre numerous times, Sam became a danger to himself.
Going out with pals twice a week, Sam's drinking started to pick up rapidly as he felt like he had to make up for lost time, and when his estranged mother died from cancer in his mid twenties, her death became a "significant trigger". "It led to an escalation, as I was grieving which resorted to me drinking at home," Sam told TheMirror.

'I went to last Ozzy gig - it was like watching his wake while he was still alive'Meanwhile, Ozzy Osbourne performed his final show on Saturday as a legendary lineup came together and entertained a jam-packed Villa Park for Back to the Beginning - and the Mirror’s Jamie Roberts has written an opinion piece on his final gig. He penned: “Three days on and theOzzy Osbourne final show effect still hasn't truly sank in. In what isa real end of an era, the Prince of Darkness took it back to the beginningfor one last time.
“It may have been the last chance for over 40,000 people to see the legendary singer, 76, front and centre where he belongs, but the day was much more than that. It was about Ozzy getting his one last hurrah, too.
“The emotion in his trembling voice, the sincerity in his thank you messages, and the sparkle in his eye as his frail body took centre stage; you could see how much it meant to him to be back out there at his belovedAston Villa's Villa Park. And it was a moment many would never have expected after years of struggles, with his advanced Parkinson's disease visibly limiting the rocker.”

'Kate's response to difficult Macron gesture after wife's snub speaks volumes'Finally, the Princess of Wales was greeted by French President Emmanuel Macron with an unusual air kiss on the hand - and a body language expert has revealed what Kate's reaction tells us. Both Kate and William were at RAF Northolt this morning to greet Macron and his wife Brigitte as they arrived in the UK for a state visit.
As the couple emerged from their jet, the French leader offered his hand to his wife as she descended the stairs, but she declined his assistance. The president then warmly shook William's hand and held onto his arm as they chatted. Mr Macron then took the princess’s hand and delivered a gallant air kiss towards it.
Body language expert Judi James told theMirror: "When his display of gallantry became distilled into a quaintly antique air hand-kiss that he seemed to have fashioned out of her offered shake. This is a difficult ritual for the modern woman to respond to as it tends to prompt an embarrassed giggling in these more feminist times.
"But Kate smiled gracefully, perhaps aware that it would be Brigitte's turn next as Charles opted to lift the back of her hand right up to his lips to maintain eye contact as he planted his own lips."
